PLEASE NOTE: With patient access to eye care services now increasing in Ontario, we are no longer accepting requests from the public for e-Assessments. If you are a non-Prism patient who requires care, please see your optometrist or family doctor.
If you do not have an optometrist, you can find one in your area on the College of Optometrists of Ontario Website.
If you are not under the care of an optometrist, you can request an appointment with a Prism optometrist HERE.
Please note that staff review incoming messages Monday to Friday from 8:00am to 5:00pm. Incoming messages are NOT reviewed on Saturdays, Sundays or statutory holidays.

What You Should Know About Your Eye Exam
To make the most of your appointment, please fill out your patient intake form before coming in.
On the day of your appointment:
- Bring a list of medications you currently use, including eye drops.
- A friend or family member is welcome to accompany you, especially for your first appointment.
- Please allow one–two hours for your appointment. Eye emergencies may cause wait times to vary.
- Please allow forty-eight hours notice if you need to cancel your appointment. Service fees will apply to cancellations within forty-eight hours and missed appointments.
What to Expect During Your Appointment
During your comprehensive eye exam at Prism Eye Institute, we’ll assess the overall health of your eyes through various tests, including visual acuity and eye pressure, and discuss your medical and vision history.
Our friendly and knowledgeable team is here to answer any questions you may have about your vision. We’re dedicated to helping educate and empower you regarding your ocular health.
Do I Need a Referral for Appointments?
You can request an appointment directly with our office for the following:
- Eye health checkups, eye health concerns, and changes in your eyesight
- Screening for diabetes, glaucoma, cataracts, and macular degeneration
- Eyeglasses and contact lenses
- Evaluations for laser eye surgery
