You can feel confident referring a patient to Prism Eye Institute. Our treatments are patient and technology-focused, with an emphasis on care and education. Our extensive network features over 2,590 registered referring doctors, and together we can help more patients receive the care they need.

We aim to make the referral process both easy and efficient. Here are our simple steps:

1. Complete and Submit a Patient Referral Form

Please fill out this Consultation Request Form. If medically urgent, please specify and ensure to send all relevant medical documents with patient.

Please allow 24 to 48 business hours to process non-urgent cases and 12 to 24 hours for urgent cases.

2. Confirming an Appointment

Appointment details will be faxed back to your office. Please advise the patient of the appointment details. To ensure efficiency, we will contact same-day or urgent patients directly.

If you have not received a fax back with appointment details after 48 hours, please call our dedicated physician line for assistance at 905-456-7201.

3. Receive Consult Reports

Consult reports will be sent out within one week of your patient’s appointment. Should you require it sooner, please call our dedicated physician line at 905-456-7201.
